It is currently not possible to insert/attach images or documents in/to individual exam questions.
This would be a great feature as it would allow reference to question-specific pictures or text and phrasing of questions accordingly. (E.g. With reference to the attached diagrams, which configuration is correct for a limited power approach... A, B, C or D?)

It's already possible to do this, there's a "Question Image" option when editing a question.
