Customize layout of checkforms

This is in reference to ticket ID AMT-2442, and creating a function to export check forms, but also to customize their appearance or layout of checkforms.

Reportable overall Competent / NYC in Check forms - at top of check form

It is a CASA requirement to be able to report on the number of failed checks. Having a field at the top of the form to say if it was a pass or failed check would solve this gap in the system.
